Her usual occupation was "housewife" in the "home." She had not served in the U.S. armed forces. She did not have a social security number. Immediate cause of death was "Acute [decompensation?].". Due to "Rheumatoid Arthrits [sic]" (17 years duration). Due to "Arteriosclerosis." There was no autopsy. Burial was to be on 24 April 1960 in "Buckner Cemetery" Buckner, Missouri. The funeral director was Reppert, of Buckner, Missouri. Personal information was provided by E. K. Knapheide, of Carrollton, Missouri. Wm. H. Goodson, M.D., of Liberty, Missouri, had attended the deceased for 20 months, had last seen her alive on 21 April 1960, and signed the death certificate on 21 April 1960. The certificate was received by the local registrar on 23 April 1960.
           A some of the personal and all of the medical information was filled out by hand, nearly all of which appears to match the signature of the attending physician. The place of death, some of the personal information, and the date and place of burial was recorded with a typewriter.

Signed by "Paul W. Knapheide" on 13 November 1907.
           Affidavit of Female: This portion of the application was left blank. In its place was a statement by "Willie J. Lane," who swore that "Miss Bessie Katheren [sic] Myers" was personally known to him and that she was of the age of 18 and legally able to marry. This affidavit was signed by "Willie J. Lane" on 13 November 1907.
